Dear patient,

There is a new patient self-referral form to access abortion care throughout Cardiff and Vale. This is to replace the previous phone line, although that number is still available to call if someone is unable to complete an online form and it is available on the website.

All necessary information about the abortion service is available on this website, but please note what is written below:

• To avoid delays in getting an appointment, it is important that you fill all demographic data requested correctly, as incorrect details often mean we cannot find/or we have to create a new patient on the system to generate an appointment.
• It can take up to 3 working days for the service to get back to you, but they will get back to you with an appointment date
• The service usually send an appointment by text, if they need to call you it will be on a withheld number.
