New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill or Passport.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. When you register you will need to complete the form and also provide evidence of identity such as a copy of your Passport. For Patients from outside of the Europe a copy of your passport and valid visa will need to be provided.
All new patients are offered a health check with a member of the healthcare team which will include a blood pressure check and urine test. The health check will ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Please ensure you attend when requested.
Medical treatment is available from the date of registration however you will be asked to provide details of current prescribed medication, usually in the form of a Repeat Prescription slip or a print out from your previous surgery. Please contact reception for further information.
Please note that the FOUR ELMS MEDICAL CENTRE does NOT routinely prescribe BENZODIAZEPAM’s (e.g. Diazepam, Lorazepam & Temazepam, ETC) If you are taking this medication when joining the practice, you WILL be asked to see our CLUSTER PHARMACIST with a view to reducing and eventually STOPPING your medication.

1. TO COMPLETE REGISTRATION PLEASE DOWNLOAD AND PRINT OFF THE FORMS WHICH MUST BE SIGNED AND RETURNED TO THE SURGERY. WE ARE UNABLE TO ACCEPT REGISTRATION UNTIL WE RECEIVE THE SIGNED FORMS. WE WILL ALSO NEED TO SEE ORGINAL EVIDENCE OF IDENTITY TO VALIDATE IDENTITY AS DESCRIBED ABOVE.
ALL NEW REGISTRATION APPLICATIONS HANDED IN TO CARDIFF ROYAL INFIRMARY WILL BE PROCESSED AT OUR MAIN SITE IN PENGAM GREEN.
IN THE EVENT OF ANY ERRORS OR OMMISSIONS YOU WILL ASKED TO ATTEND PENGAM GREEN TO CORRECT THESE.
PLEASE AVOID BUSY PERIODS WHEN REGISTERING - FOR YOUR CONVENIENCE WE PREFER TO PROCESS NEW REGISTRATION REQUESTS BETWEEN:
11:30AM – 4:00PM
ON RECEIPT OF ALL SUPPORTING DOCUMENTATION ALL NEW REGISTRATIONS WILL BE COMPLETED WITHIN 5 WORKING DAYS.
IF YOU WISH TO SEE A DOCTOR WITHIN THIS TIME PLEASE CONSULT YOUR EXISTING GP AND ENSURE THAT YOU HAVE SUFFICIENT MEDICATION TO LAST THROUGHOUT THE CHANGE OVER PERIOD.
WE CANNOT ACCEPT INCOMPLETE FORMS. THESE WILL BE RETURNED TO YOU FOR COMPLETION.
